The short version, three steps
Check with your plan first
Call the number on your benefits card and ask two things: is this equipment covered, and does your plan need to approve it before you buy. Insurers call that approval pre-authorization or a predetermination. Your plan's website or app can answer this too.
Get the required medical documents
Ask your insurer what medical information or form it needs and who must complete it. That may be your doctor, another prescriber or an occupational therapist. If pre-authorization is required, send the clinician documents with a written estimate from us and ask the insurer to confirm coverage and the approved amount in writing.
Buy, then claim
Pay for your order, then submit your itemized receipt and prescription through your plan's website, app, or by mail.

Pacific Blue CrossMay need pre-authorization first
Pacific Blue Cross lists wheelchairs and similar equipment among the items that may need pre-authorization. They ask for the same paperwork as the claim itself: a doctor's note, a written estimate, and sometimes an occupational therapist's assessment for wheelchairs. Request our estimate here.

Canada Life's healthcare claim form has an option for submitting an estimate before you buy. Request the written estimate here and send it in the same way you would a claim.
What to collectTheir healthcare claim form asks for the patient's name, the date the item was received, an item name or detailed description, the charge for each item, the provider's name, address and telephone number, and any amount paid by a provincial plan. Care Made can provide the item and retailer details. Ask us if the patient name or date received needs to appear in a particular way, and confirm any remaining requirements with Canada Life.

Green Shield's authorization form for durable medical equipment goes in before you purchase. The clinician completes the medical and prescribed-item sections, and Care Made can provide the estimated cost for each item. Request our estimate for the costs.
What to collectA doctor's prescription showing the diagnosis and what is prescribed, plus an original itemized paid receipt. Green Shield asks that receipts show your name, address and Green Shield ID number, the provider's name and address, the date and charges, and a detailed description of the equipment. Their claim forms also note that claims must be submitted within 12 months of the date of service, unless your plan documents say otherwise.

Sun LifeConfirm coverage in writing first
Sun Life publishes less equipment detail publicly than the other insurers here, and coverage varies by plan, so the sure route is asking them directly through my Sun Life or by phone. Request the written estimate here and send it with your question.

For equipment over $500, Manulife recommends sending them a cost estimate to review first. Request it here and we will prepare it.
What to collectA doctor's referral stating why you need the equipment, how long you will need it, and the activities it will be used for, plus a receipt showing the patient's name and the date of purchase. If your provincial plan contributed, include its statement. If the item replaces older equipment, tell them how old the old item is and why it is being replaced.

Medavie says approval is required before purchase and asks for two quotes from different providers. Care Made can provide retailer and product details plus an itemized cost breakdown. Government funding works differently
Government programs such as Ontario's Assistive Devices Program, Veterans Affairs, WorkSafeBC and provincial ministry programs have their own eligibility, approval and vendor rules. Some require purchase from an authorized vendor or approval before ordering, and an online Care Made purchase may not qualify.
